Appointments

If you’d like to make an appointment with one of COJIs Orthopaedic Speciaists and prefer not to submit a request through our online form above, please call the office Monday thru Friday at 352-746-2663 (BONE).

While a referral or request from another physician IS NOT required to be seen by our providers, please have the name of the physician available if another doctor did indeed refer you to see us. We will also need standard demographic information, including an email, insurance information, and the history of your present ailment. If your insurance requires a preauthorization, we will do our best to help you obtain the authorization from the insurance company or referring doctor as appropriate. If any X-rays or other advanced imaging procedures were performed at another facility prior to your appointment, we will need the actual study ( usually on disc) as well as the written interpretation from the doctor prior to you being seen.

While we are contracted with most insurance companies, there are some with which we are not. HOWEVER, we make every effort to work with patients and their insurance company so that you can still reap the benefits of the excellent care our physicians provide, while not becoming financially overwhelmed.
